Major pharmaceutical firm, Merck & Co., brought Propecia to market in 1997. This alopecia (“hair loss”) drug was a novel application of the compound finasteride, which was first synthesized in 1992.

Finasteride received close to $500 million in funding and was released to great applause. Within the USA alone, it attracted 400,000 daily users in its first year of release. Merck made it known that Finasteride was shown in medical testing to have certain adverse effects upon the male reproductive system. However, it was subsequently discovered that they had downplayed this information.


Once Propecia gained FDA-approval, Merck revealed that 3.8% of users had experienced at least one incident of impaired sexual function. Contrasting this figure against placebo trial results, in which 2.1% of participants experienced similar problems, revealed a small but undeniable side effect. The major finding which Merck suppressed was that this negative consequence of Finasteride could be permanent. The company initially claimed the effect would fade once usage of the drug was stopped.


Proscar and Propecia are both classified as 5-alpha-reductase inhibitors. Such drugs have application in the treatment of disorders caused by DHT, or Dihydrotestosterone. This powerful sex steroid and androgen hormone is produced in the body from regular testosterone. DHT can give rise to such problems as BPH (enlarged prostate gland), cancer of the prostate gland and MPB (alopecia).


In men who inherit MPB, Dihydrotestosterone binds with the hair follicle receptors found atop the skin of the head. DHT’s action upon these hair follicles shrinks them over time, so that they produce thinner, weaker hair. The hair on top of the scalp is more commonly affected than the back and sides. This is what causes the characteristic thinning hair, expanding bald spot and receding hairline of MPB. There are roughly 35 million men in America who have this condition.


Finasteride, the active component of Propecia, counteracts MPB by sharply reducing levels of DHT. The body’s level of DHT contracts by 65% after only a day of taking the substance. However, the observable result of this reduction in DHT may take 3 months or longer to fully manifest.
